Home-based primary care (HBPC) is a healthcare design in which the practitioner travels to the patient. It is commonly called “house calls,” Chronic medical conditions often deteriorate with age and many patients suffer from functional impairments that make it hard for them to access medical care. This challenge is even larger for those with little financial means or social support such as a family member to take them to appointments.
Home-based primary care provides a way for these patients to receive comprehensive medical and social care in their homes. When a physician, nurse practitioner or physician’s assistant visits the home, patients are less likely to need emergency room visits or hospitalizations.
